How often   Quarterly requirement
 
Due date   One month after calendar quarter-end: April 30, July 31, October 31, January 31.
 
Workers exempt from UI and DI?   Students working for their schools, life insurance agents and family members are exempt from disability and unemployment insurance. Do NOT include any of their wages on form with your other employees unless they choose to participate in the disability insurance program.
 
Corporate officers   Sole shareholders must submit the Sole Shareholder Exclusion Statement prior to claiming an exemption from disability insurance.

    Please note that the total personal income tax withheld that you report on this form should match the total personal income tax that you deposited during this quarter using DE-88.
